CHECK IN

For your first appointment, please arrive 15 minutes early to check-in and complete all necessary paperwork.  Always bring your insurance card(s) and photo identification to every visit with Transcend Medical Group. We appreciate your cooperation because it enables us to process and file information about your visit more efficiently.  Your time is valuable, and we want to help you make each visit as pleasant as possible. The following tips and guidelines will help you help us take care of you more efficiently.  Our physicians and staff try to simplify the registration process and make every effort to minimize your wait.

Please bring the following to your first appointment:

1. Photo identification card or driver license card

2. Insurance card

3. All current medications or a list of your current medication list which include the following (Medication Name, dosage, and direction)

4. Recent Lab Results report or imaging

 

After you arrive at our facility, you will:

Check in with the front desk to receive your registration paperwork.  Our staff member will request your driver's license or other government-issued I.D., and your insurance card.

Return your registration forms to our staff member.  You will be seen by a doctor in the order in which you arrive.  After you have been seen by the doctor, and have received any other tests, vaccines or medical services, you will check out with a member of our staff.  At this time, you will be responsible for payment of the services rendered at today's visit.

CANCELLATIONS

Our office provides an automated appointment reminder call one business days prior to your scheduled appointment as a courtesy to you. If you need to cancel or reschedule an appointment, please call 817.860.2700.
